How to Prepare for Your Virtual Consultation

You don’t need to put your dental care on hold. ClearChoice Dental Implant Center of Garden City is now offering free virtual consultations, so you can have a one-on-one conversation about dental implant treatment options from the privacy and comfort of your own home.

This is an important step in restoring your dental health, so we’re providing instructions and information on how to get the most out of your virtual consultation with ClearChoice.

What to Expect from Your Virtual Consultation

During your virtual consultation, you will:

  • Discuss your dental issues and treatment goals with a ClearChoice doctor
  • Learn more about a solution that looks, feels, and functions like real teeth
  • Review the dental implant process at our all-in-one center
  • Get your questions answered
  • Go over typical cost ranges and third-party financing options

3 Steps to Set Up for a Successful Virtual Consultation

In preparation for your virtual consultation with ClearChoice, please complete the following 3 steps at least 24 hours before your appointment. Instructions for each step are below.

Step 1: Take Your Dental Photos

Taking a few photos of your teeth and uploading them to our secure platform will help inform the discussion about your dental issues.*

Grab your smartphone, find an area with good lighting, and ask a family member or friend to help you take the photos.

Remember to wash your hands before touching your face or mouth. Also, if you wear a removable dental piece (like a denture, partial, flipper, or snap-on veneers), please remove it before taking your photos.

The video below will walk you through how to take photos of:

  • The front view of your teeth
  • Your upper teeth
  • Your lower teeth
  • If applicable, your removable dental piece (denture, partial, flipper, or snap-on veneers, etc.)

Focus in your photos is important–please review your photos to ensure that they are sharp and clear, and focused on your mouth and teeth. Avoid taking and submitting full face photos.

With a slight space between your top and bottom teeth, smile and pull your lips and cheeks back as much as possible. Use your fingers to help get a clear view of your teeth. Take a close-up photo.

Now, open your mouth as wide as possible and tilt your head back. The angle of the camera should be from below, to see all upper teeth. Take a close-up photo.

Next, open your mouth as wide as possible and tilt your head down. The angle of the camera should be from above to see all lower teeth. Use your fingers to pull back your lips for a more complete shot. Take a close-up photo.

If you wear a removable dental piece like a denture, partial, flipper, or snap-on veneers, take a photo of it in your hand.

Step 2: Upload Your Dental Photos

You should now have 3-4 photos taken that are in focus and show clear views of your teeth. Please fill out the form below, then select “Browse files” to add your photos. You may add them one at a time. When you have all photos listed in the box and a fully completed form, select “Upload” to securely send them.

*By uploading and transmitting your photos, you consent to the use of such photos by the ClearChoice doctor(s) and clinical staff for purposes of preliminary discussions about your dental condition. A conclusive diagnosis or individualized treatment plan cannot be provided during the virtual consultation. After the virtual consultation, a no-cost, in-person visit can be scheduled (including diagnostic imaging such as an iCat scan) to confirm your diagnosis and determine a final treatment plan. No dental treatment will be performed until a full in-person tactile and visual examination has occurred.

These risks include, but may not be limited to, the following: in rare cases, the practice staff may determine that the connection is of inadequate quality, thus necessitating a face-to-face meeting with you or at least a rescheduled virtual consultation; delays in medical information can occur due to deficiencies or failures of the equipment; all vendors used by the practice in providing technology are required to comply with the requirements of HIPAA, however, in very rare instances, security protocols could fail, causing a breach of the privacy of personal medical information.
